Providence Health & Services Outpatient Behavioral Health - LCSW or CSWA in Hood River, Oregon
Providence is calling for a full-time Licensed Clinical Social Worker (or CSWA) to Providence Hood River Memorial Hospital in Hood River, OR.
This position will support the Behavioral Health Outpatient Clinic & the Chemical Dependency Clinic - running group sessions and individual counseling sessions. This is a great opportunity to help rebuild the Chemical Dependency program and development new group work curriculums!
Coverage will be four 10-hour shifts, generally between 8am - 6pm, Monday through Friday. Occasional coverage for evening and weekend telephonic support for patients in crisis (work from home for these on-call shifts).
Preference to LCSW, but CSWA are encouraged to apply!
In this position, you will:
Independently provide comprehensive biopsychosocial/spiritual assessment, diagnosis and/or treatment/interventions/advocacy of patients and their support systems.
Incorporate concepts and theories associated with life span development, neurobiology, and behavioral change.
Collaborate with the patient and support system to develop and implement evidence-informed person-centered care plans across the care continuum in collaboration with the Inter/Multi-Disciplinary Team (IDT).
Perform duties in a manner that respects patient self-determination, that reduces health disparities/barriers to care, and adheres to the National Association of Social Workers (NASW) code of ethics, Standards of Practice (SOP), and mission.
Required qualifications for this position include:
Master’s degree in Social Work from a Council of Social Work Education (CSWE) accredited school of social work, or from a school in candidacy status by the CSWE
Licensed Clinical Social Worker (LCSW) with the state of Oregon
Licensure as an Licensed Independent Social Worker (LICSW) in the state of Washington as needed based on work location(s)
Advanced Certified Hospice and Palliative (ACHP) Social Worker certification preferred, or must obtain within 2 years of hire.
Valid driver’s license and reliable transportation
2 years of relevant social work experienceMay include CSWA experience
Ability to assess biopsychosocial/cultural and spiritual in facilities and homes.
Ability to assess healthcare literacy of the patient and their family members.
Ability to diagnose and treat depression and anxiety related to a serious and life limiting illness.
Ability to give counseling and education related to end of life planning.
Experience working with individuals with a serious illness
