Providence Health & Services Participate Support Aide in Hood River, Oregon


Providence is calling a Participant Support Adult Day Services Aide - Down Manor Columbia Gorge Service Area (0.8 FTE, Days) to Brookside Manor Hood River in Hood River, OR.

We are seeking a Participant Support Adult Day Services Aide - Down Manor Columbia Gorge Service Area that will be responsible for assisting with planning, organizing and implementing all program sponsored activity programs for the participants of Providence Senior Village-Sunshine Club. Will assist with ensuring the program is operating within guidelines form the Department of Human Services Senior and People with Disabilities Chapter 411 Division 66. Assist with assessment, service plan, documentation, and responsibility for participants in the program.

In this position you will have the following responsibilities:

  • In conjunction with Activities Coordinator, schedules transportation for participants

  • Cue or provide assistance with ADLs for participants as noted in care plan

  • Work closely with the Senior Village staff better promote the facilities and program and to present Senior Village as a desirable place for seniors to live

  • Help make Senior Village / Sunshine Club an enjoyable place for residents/participants to be

  • Ensure that the area is well maintained, stocked and set up for planned activities and resident needs

  • Assist with the implementation of activities throughout the day

  • Adhere to safety guidelines and can manage resident illnesses or accidents that occur during activities

  • Assist with assessment of each resident upon starting the program and ensure there are appropriate for the program

  • Assist with managing care plan, reviews, and documentation per regulations

  • Assist with Meal planning and meal prep

  • Assist with activity planning and documentation

  • Assist with documentation as outlined in the rules and regulations

  • Complete ongoing continued education as scheduled


Required qualifications for this position includes:

  • High School Diploma -OR- GED or equivalent

  • Cardiopulmonary Resuscitation (CPR) and First Aid Certified (or training will be provided by employer within 30 days of hire date)

  • Valid driver's license and driving record consistent with GOP Safe Driving Policy 330 Incident Point Matrix at the time of hire

  • Two (2) years experience in working with Dementia Care -OR- Memory Care

  • Experience in planning care for individuals communication with care providers, and family members

Preferred qualifications for this position includes:

  • Certified Nursing Assistant (CNA) experience

  • Companionship, housekeeping and meal prep experience in a home setting
