Providence Health & Services Pharmacist - Outpatient Infusion Oncology in Hood River, Oregon


Providence is calling a Pharmacist - Outpatient Infusion Oncology (0.9 FTE, Rotating/Variables) to Providence Hood River Memorial Hospital in Hood River, OR. This position's shift is 4 9-hour shifts M-F from 7:30am to 5pm. The day off will either be Monday or Friday!

We are seeking a Pharmacist - Outpatient Infusion Oncology that will enter orders into Pharmacy management program, verify orders entered by house supervisors, provide patients with medication education, fill drug orders, monitor patient drug therapies and provides drug information. A Staff pharmacist will supervise and direct supportive personnel. In the absence of the pharmacy manager, a staff pharmacist may be required to assume the essential responsibilities and perform the duties of the pharmacy manager.

In this position you will have the following responsibilities:

  • Prepare and dispense drug orders per physician request according to established policies, procedures and protocols:

  • Interpret drug orders (verbal and written) and transcribes to computerized patient medication profiles accurately

  • Maintain accurate, complete pharmacy patient medication profiles

  • Compound and dispense pharmaceuticals including sterile products, chemotherapy and parenteral nutrition products accurately

  • Issue controlled substances to patient care areas and maintains records as required by law

  • Ensure safe, appropriate, cost effective drug therapies for patients according to established policies, procedures and protocols:

  • Monitor drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies and appropriateness of drug and dose

  • Assist with pharmacokinetics consult service and drug dosing per hospital protocol

  • Review/interpret culture and susceptibility data for antibiotic appropriateness and recommend changes as needed

  • Read, extract and interpret information in patient charts accurately

  • Detect and report suspected adverse drug reactions accurately and in a timely manner

  • Sustain the hospital drug formulary, minimizing nonformulary procurements, utilizing therapeutic substitution protocols and promoting rational drug therapy selection

  • Provide clinical consultation and clarification to practitioners

  • Suggest appropriate, cost-effective therapeutic alternatives to medical staff as needed

  • Provide accurate, adequate and timely drug information to the hospital’s professional staff

  • Provide drug education to patients and their families

  • Document all clinical activities and interventions accurately and completely

  • Participate in the quality improvement and medication use review activities of the department

  • Collects data, conducts monitors and inspections and maintains logs, records and other documentation as assigned

  • Contribute to the quality and effective operation of the department:

  • Work independently with minimal supervision

  • Organize and prioritize work assignments

  • Ensure pharmacy services are provided in a timely manner

  • Complete all assigned medication storage area inspections in a timely manner

  • Identify and replace outdated and unusable drugs

  • Maintain competency required for current job title/position:

  • Maintain current pharmacist license

  • Ensure applicable CE records and licensure are maintained in department files

  • Attend pharmacy staff meetings

  • Attend orientation, education and training programs

  • Review literature and other materials pertinent to the practice of pharmacy

  • Complete all competency/skills assessment requirements


Required qualifications for this position includes:

  • Pharmacy degree from an accredited school of Pharmacy

  • Licensed by the Oregon State Board of Pharmacy

Preferred qualifications for this position include:

  • One (1) or more years of Infusion/Oncology pharmacist experience
