Providence is calling RN - Hospice of the George (0.8 FTE,Variable) to Providence Hood River,OR.
We are seeking an Registered Nurse – Hospice of the Gorge under the direction of the Hospice Clinical Supervisor, and on order of the physician, this position is accountable for direct and indirect quality care for patients and families of all ages, according to the individualized needs of the patient, following the nursing process and in accordance with the Nurse Practice Act. Accepts professional development as an integral part of nursing practice. Manages calls made by patient/family and makes home visits as needed to assess needs/concerns within hospice service area. Coordinates patient care activities with other members of the health care team in a collegial relationship to promote patient comfort and assist in the achievement of a comfortable and peaceful death.
In this position you will have the following responsibilities:
Take responsibility for evening/night/on-call duty as scheduled and consistent with Providence Hospice policies and procedures
Respond to medical or emotional crisis of patient or family, and makes home visit to direct care as appropriate
Make home visits to assist patient, family or caregiver with any medical or psychosocial issues including pain and symptom management
Respond to any medical crisis involving Emergency Room visits, falls, hospitalization or placements, and makes visit to coordinate with physician and staff to assess, plan, and implement plan of care
Perform medical procedures and treatments according to accepted organizational policies, procedures and community standards
Monitor physical care of patient by instructing caregivers and volunteers
Admit patients in crisis to Hospice, contact physician, secure medications orders, and initiate plan of care
Maintain up-to-date records to guarantee coordination of care
Evaluate on an ongoing basis the patient and family's psychosocial, spiritual, and physical needs, including the need for respite care. Initiates necessary referrals to all disciplines in a timely manner
Teach, supervise, and counsel the patient and caregivers regarding patient care in accordance with the wishes of the patient and their family including cultural and religious practices, emotional barriers, desire and motivation to learn, physical and/or cognitive limitations, communication styles, and language barriers
Maintain patient clinical progress, and itinerary records to meet The Joint Commission and Federal regulations
Properly documenting nursing care in a timely manner as required by state and federal agencies to ensure proper financial management
Required qualifications for this position include:
Education to meet certification, license or registration requirement
2 years of Medical/surgical experience, recent acute care or long-term care experience
Experienced in End of Life care Criminal Background clearance
Current unencumbered Oregon RN licensure
In addition, a current Washington state RN license, or WA state interstate endorsement within the first 3 months of employment with Providence
Current BLS (Basic Life Support for Health Care Provider) or ACLS (Advanced Cardiac Life Support) required within 30 days of hire
Valid driver's license
Driving record consistent with GOP Safe Driving Policy 330 Incident Point Matrix at time of hire
Preferred qualifications for this position include:
- Recent Oncology or Home Hospice or Home Health experience
